One of the most challenging parts of the recovery process that most patients and clients experience: The Setback(s). No matter how many times I try to have the conversation early on in the rehab process, it creates a lot of noise and disrupts the system when it inevitably happens. Picture this: you are recovering from back and hip pain and are anxious to get back to training in the gym again. You have been feeling pretty good for the last week or two and decide that this Friday night you are going to venture out with friends for dinner, drinks and dancing. A couple of margarita's later, you're dropping it low to some Ying Yang twins, having the time of your life. Fast forward to the next day: you're sore...WAY more sore than you anticipated. Everything hurts! "How could this happen?! I was doing so great", you think to yourself. "Am I back to square one?! I have worked so hard to get here." Panic ensues...and I get to hear ALL about it at our next visit.
I've had countless conversations like this in my 7 year career as an ortho/sports physical therapist. It isn't always the dancing that does it, but you get the point. Don't take this as lack of empathy either, I actually really enjoy having these conversations with clients. It allows them to reframe and see the big picture. I'll use milestones and constructive feedback to help them understand how far they've come. It really is a learning experience for both of us!
Recovery from injury or surgery can be a challenging process, and setbacks are a normal part of the journey. It's important to understand that setbacks are not indicative of failure or lack of progress. In fact, setbacks can provide valuable information about what your body needs and how to adjust your treatment plan.

By sharing your concerns, frustrations, and any changes in your symptoms, you can work together as a TEAM to adjust your treatment plan and address any underlying issues that might have lead you to this point. Remember, your physical therapist is your ally in your recovery journey, and by maintaining open lines of communication, you can better manage setbacks and continue progressing towards your goals.
Effective communication is crucial in managing expectations and setbacks during the physical therapy recovery process. It starts with setting realistic goals and understanding that the road to recovery is seldom linear. By discussing your goals and expectations with your physical therapist early on, you can establish a shared understanding of what success looks like for you and how setbacks will be managed.
Throughout your recovery, maintaining open lines of communication with your physical therapist is essential. If you experience a setback or notice any changes in your symptoms, it's important to inform your therapist as soon as possible. By providing timely and accurate information, your therapist can adjust your treatment plan accordingly to address the setback and help you get back on track. I am amazed at how often I have to pull information from clients that should have been a conversation earlier on. If in doubt, don't wait or hesitate - reach out!
In addition to communicating with your physical therapist, it's also important to communicate with yourself. Be honest about your limitations and listen to your body. Pushing yourself too hard can lead to setbacks, so it's important to find a balance between challenging yourself and allowing for proper rest and recovery. You need to keep in mind that the body loses tolerance to certain movements, activities and positions after an injury and that just because something "feels good" doesn't mean you should go all in! Take things slow and cautiously, especially in the beginning. I can't tell you how many times I have had this conversation with clients who felt good on their run, so they decided to keep running, only to be sore and upset the next day wishing that they had stopped earlier.
Overcoming physical therapy challenges requires practical tools to navigate setbacks and continue making progress. Here are some strategies that can help:
- Stay consistent with your prescribed exercises and home program. Consistency is key to building strength and improving mobility. Even on days when you may not feel motivated, try to complete your exercises as prescribed.
- Break down your goals into smaller, achievable milestones. This can help you stay motivated and focused on making progress, even when faced with setbacks.
- Utilize modifications as needed or as prescribed by your provider. Your physical therapist can provide guidance on making modifications to your environment to help you overcome specific challenges related to activities. This strategy helps us create an entry point back into the activities that are meaningful for you (ie; we prescribe box squats instead of full depth so you can continue squatting!)
- Practice self-care and prioritize rest. Recovery takes time, and it's important to give your body the rest it needs to heal. Make sure to incorporate rest days into your routine and listen to your body's signals.
- Seek support from friends, family, or support groups. Having a strong support system can help you stay motivated and provide encouragement during challenging times. There are lots of groups that exist on social media and other forums that where individuals who have overcome common injuries provide their experience and feedback. They are a great resource with a powerful community of athletes to help guide you through the process of pre-surgery all the way to putting your cleats on again and stepping onto the field.
